Irrigation Account Leak Bill Adjustment Application

Please read the following instructions carefully.

The following conditions are required to complete this application:

  • You must be the account holder, or the account holder's agent, to make this application.
  • You must have the utility bill for which you are seeking a leak forgiveness. You will need to reference the bill in order to complete the application.
  • You must read, understand and agree with the Leak Adjustment Policy before completing this application.

Instructions

  1. Read, understand, and agree with the Leak Bill Adjustments Policy. The policy is set forth under the authority of Municipal Code 16.13.050 and cannot be altered, expedited or otherwise deviated from by SCMU staff.
  2. Answer the application questions completely. When asked for detail describing your leak, please provide as much information as possible.
  3. Select Next Page after answering every question on the page.
  4. Select Submit upon completing the application. If needed, SCMU will contact you to request further information. Allow 4 - 6 weeks for application review and processing. 

If, after completing this application, you have questions regarding the Leak Bill Adjustments Policy, the Municipal Code, or your submission, please email scmu@santacruzca.gov with your name, account number, and a brief description of your concern. A customer service representative will contact you with a response.
